Where Do I Start?

If you are thinking about exploring long-term care options and the care you will need in the years ahead, Home and Community Care Support Services (HCCSS) can help. You can apply to the Homes by contacting the Home and Community Care Support Services (HCCSS). A care co-ordinator from the HCCSS will arrange to meet with you to complete all of the required assessments.

Service Description:

  • Access to health care support at home, at school or in the community
  • care coordinators work with people in the community or leaving hospital to determine what help is needed and arrange for care
  • care is provided in community settings such as the home, school, residential facility or HCCSS community clinic
  • provides information about other community services and refers when appropriate
